I am applying for employment with Residential Support Services, which provides services for people with disabilities. I understand that in order to make a knowledgeable decision regarding my application for employment, the administrative staff at RSS must contact my references. I, therefore, authorize RSS to investigate my past record and to ascertain any and all information which may concern my record and character, whether same is of the record or not, and I hereby release all persons whomever from any damage because of furnishing this information. I understand that contacts may be made with any of the following employers, courts, law enforcement agencies, social services, and any other persons or agencies I have had contact with. To the best of my knowledge, the information I have provided and the statements I have made in this application are correct and complete. I understand that misrepresentation or omission of facts called for in this application may cause for immediate dismissal. I understand that my employment is contingent upon satisfactory employment and personal references and a satisfactory background check.
